Designate A Beneficiary for Your Travel Accident Insurance

You can designate a beneficiary for your Travel Accident Insurance coverage to ensure benefits due to loss of life are distributed according to your wishes. If no beneficiary is assigned, the benefit is payable to your estate. Follow these steps to select a beneficiary:

  1. Click here to download the Beneficiary Designation Form.
  2. Print out and complete the Beneficiary Designation Form.
  3. Sign the form. For your protection, a signature is required for the beneficiary selection to be valid.
  4. Mail or fax to the Travel Accident Insurance Plan Administrator.
